To Post Acute Specialists,

Sail Healthcare is a group of respiratory care practitioners who care for high complex vent/trach patients at home requiring mechanical ventilation, non-invasive ventilation (NIV), and High Flow Nasal Cannula (HFNC). All of our patients are required to be followed by a pulmonologist outside the hospital. Because many of our patients are 24/7 ventilator dependent, the caregivers find it difficult for a pulmonologist to accept them in their clinic or office settings. There are very few clinics that will accept these patients, and the waiting list to be seen by a pulmonologist can be up to 6 months.

Sail Healthcare has found a solution for our patients who require pulmonary services following hospital discharges. We are using Post Acute Specialists to see our patients via Telemedicine during our respiratory care practitioner home visits. By us seeing a patient with a physician, NP, or CNS together we can develop a respiratory plan to keep these patients home safely and prevent hospital re-admissions. Post Acute Specialists are able to see our patients within 48 hours following hospital discharge.

Belterra Health & Rehabilitation facility has enjoyed partnering up with Post Acute Specialists to deliver exceptional case to our residents this year. While Post Acute Specialists offer multiple specialty disciplines, we have utilized both Neurology and Orthopedics. We are adding Podiatry this month.
